Introduction: The United Nations (UN) recognizes Small Island Developing States (SIDS) as a heterogenous group with common geographic and socio-economic challenges. Their vulnerability to disasters was exacerbated during COVID-19 because of emerging nationalism protectionism towards supply chains resources. This study aimed determine if multilateralism engagement improved outcomes Foreign Affairs investment short term outcomes. Method: Metrics were developed reflect country parameters, clinical impact COVID-19, in multilateralism, health systems strength integration the International Political Economy. Open-source information used quantify proxy measures calculated percent spent on foreign affairs being major disaster lag time vaccination, case burden deaths first six months 2020. Data collected for each 38 SIDS. SPSS assess possible correlations short- long-term pandemic. Results: SIDS noted have an average 4.1 begin vaccination rollout compared nations prolonged below global rates. Expenditure reduced (p=0.03), decreased short-term cases (p<0.001) (p<0.001), Human Development Index rates lowered total (p=0.03). Direct Investment (FDI) inflow also (0.02).
